Your First Visit

When you first arrive at Trusted Dental Care, you’ll be greeted warmly by our team and taken on a tour of the dental office – we want to help you feel comfortable here right away! Then, it will be time to sit down and meet Dr. G for your comprehensive exam. He will review your current oral health, answer any questions you may have, and help you determine if further treatment is needed at this point in time. We want every patient to leave feeling confident about their smile’s better, brighter future.

Patient Forms

New patient paperwork is an essential part of visiting a new dentist – thankfully, we’ve made it easier to get a head start on completing this step before you ever even set foot in the dental office. Follow the links below to download/fill out online the necessary files at home. For the privacy forms, you can bring the completed versions to your appointment and turn them in directly to our front desk team, which will help us welcome you back to the treatment chair even faster. The registration and medical history forms will be automatically submitted online upon completion.
